The Coca-Cola Community All-Americans program is all about recognizing, celebrating and applauding intercollegiate student-athletes who are making a difference in others' lives through community service. Please provide a brief description of the student-athlete's community service activities while enrolled at your university or college only. Responses must be limited to 400 words or less.
List the student-athlete's volunteer activities and estimate hours spent in service per month.
Describe the impact of the student-athlete's contribution(s) on the local community or non-profit organizations.
Provide at least one example of your student-athlete's community service activities that had a significant or compelling impact on those being served (i.e success story).

Supporting Materials:
Nominators are encouraged to submit support materials, including photographs and/or videotapes of their nominee's service involvement. This material will not be a determining factor in the selection process but instead will be used to promote the program and the goodwill of student-athletes. Supporting material will not be returned.

Rules:
Only one student-athlete may be nominated per school and per form.
Nomination forms must be received by a date to be determined in order to be considered by the national review committee.

Review and Notification Process:
All nominations will be reviewed and screened by the NACDA review panel. If a nominee is selected as a Coca-Cola Community All-American, the NACDA staff will contact the nominator. The nominator will then assist the NACDA staff in determining a local event at which to present the award to the honoree.

The nomination information should be completed in the following way:
1. Complete the online National Nomination Form via Coca-Cola Community All-Americans website (www.nacda.com/cccaa)
2. All supporting materials (i.e. photographs and videotapes) should be mailed to:
